Primary care, without the rush.
Direct primary care by telehealth for adults in Virginia, Maryland, West Virginia, Indiana, and Ohio: one physician, unhurried video visits, and direct messaging, for $99 a month.
Direct primary care (DPC) is a flat monthly membership with your physician instead of per-visit billing through insurance. You get time, access, and continuity: the same doctor every visit, and answers between visits without a portal queue.
What’s included
- Same- or next-day video visits for new concerns
- Unhurried visits for chronic conditions such as high blood pressure, diabetes, high cholesterol, and thyroid disease
- Direct messaging with your physician between visits
- Preventive care planning and screening recommendations
- Medication management and prescription refills when appropriate
- Referrals and coordination when you need in-person care, imaging, or a specialist
What isn’t included
Labs and medications are billed separately, and both are available at discounted pricing through the practice. Emergency care, hospital care, imaging, and specialist visits are outside the membership.
How it works
- Book a consult on OFQ Health to see whether the practice is a fit.
- Join for $99 a month. No contract, cancel anytime.
- First visit: a full review of your history, medications, and goals, plus any labs worth ordering.
- Ongoing: visits when you need them, messages when you have a question.
Do I still need insurance?
We recommend keeping health insurance. A direct primary care membership covers your primary care relationship, not emergencies, hospital stays, surgery, or specialist care. Many members pair DPC with a high-deductible plan and an HSA.
What it costs
- Membership: visits and physician messaging, cancel anytime
- Labs and medications are not included; both are available at discounted pricing through the practice
- HSA funds and pre-tax dollars can help; see below
- Adults 18+ located in Virginia, Maryland, West Virginia, Indiana, and Ohio
Use your HSA. You should.
Starting in 2026, federal law lets you pay a direct primary care membership with HSA funds, for memberships up to $150 a month. Javid’s Primary Care membership is $99, so you can pay for it with pre-tax money. Prescriptions and lab work ordered for your care are generally HSA- and FSA-eligible too.
Eligibility depends on your plan and how each service is used, so confirm with your HSA administrator or tax adviser. The Longevity & Wellness and Psychiatric Care memberships may not count as direct primary care under IRS rules.
